Stroke Non Hemoragic biasa disebut dengan stroke iskemik yang muncul akibat gangguan pembuluh darah yang dapat menyebabkan resiko perfusi serebral tidak efektif yang mempunyai arti beresiko mengalami penurunan sirkulasi darah ke otak. Studi kasus ini bertujuan untuk mengetahui Asuhan Keperawatan pada Ny.R dengan diagnosis keperawatan resiko perfusi serebral tidak efektif dalam tinjauan teori 21 masalah keperawatan Faye Glenn Abdellah di Ruang Melon RSUD Madani Provinsi Sulawesi Tengah. Laporan ini menggunakan metode deskriptif dengan cara pendekatan dalam proses keperawatan. Data dikumpulkan dengan menggunakan format pengkajian, analisa data, intervensi keperawatan, implementasi serta evaluasi pada Ny.R Dari hasil pengkajian didapatkan tubuh sebelah kanan klien melemah, klien mengatakan nyeri pada kepala, klien mengatakan mempunyai riwayat hipertensi rentang gerak menurun, kekuatan otot ekstremitas atas dan bawah sebelah kanan melemah adalah 4, kesadaran compos mentis, GCS E4M6V5TTV : Tekanan darah 160/93, Nadi 100x/m, Pernapasan 22x/m, Suhu 37,0?C, SpO2 : 97%, ada riwayat hipertensi dan korestrol yang tidak terkontrol. Implementasi yaitu memberikan posisi head up atau meninggikan kepala 30?, evaluasi keperawatan pada Ny. R masalah resiko perfusi serebral tidak efektif teratasi.
